I've actually been wondering about that too. Perhaps it's one of his multiple personalities that's existed all the time, and been kind of hiding, and now it "returns". I don't know...
I know I sound like Sysiyo when I say this, but shouldn't this be in the interpretation? Don't know, just a thought.

S2S is the first completely conceptual album of the catelog. If you look at the YA video, he is qute thin, due to his low life condition at the time. This is just a observational quess, but he probably was introducing duke at that phrase in an attempt to make something creative out of a bad time in his life. He knew of duke back thin and that's all that matters. Also, the phrase adds to the mystery that is a Bowie lyric and it rhythmically fits.

The Duke last appeared as Ziggy Stardust. He was now returning to the stage in his next world tour.

"The return of the Thin White Duke" ranks among the best Bowie and non-Bowie album opening lines ever.
I always liked the phrase because it connotes a sense of exile prior to the Duke's "return," as though he had spent some of the recent past thinking "I am the once and future king" and now he was acting on his desire and destiny to be the current king.
Oddly, a couple albums later, on "Heroes," Bowie sings "I will be king"--and it is now in the future tense. I guess our Duke was dethroned again, but still had aspirations of retaking the throne.
